If you forgot your User ID, you can retrieve it by entering your Social Security Number. If you don't want to retrieve your User ID, click the 'Cancel' button.

If you enter your SSN and do not receive a prompt for your Secret Question and Answer, please contact MVP Plan Administrators, Inc. at 1-866-687-6877 or mvp@mvpplanadmin.com to obtain your login information. Our system cannot verify your identity and therefore, will not send you any login information. The reason may be because we do not have a valid email address saved in our system and/or you did not initially login to the system and create a secret question and answer.

Once you have created a secret question and answer, this link may then be utilized. If ever you forget your login information in the future, you can click on this link, be prompted to provide the answer to the secret question you created, and if answered correctly, your information will be emailed to you.

Once you obtain your login information and have successfully accessed your account, go to the top menu item 'My Profile' then click on 'Personal Information'. Make sure we have your correct email address, if we do not, please update it in this section. Then go back to the top Menu 'My Profile' and click on 'Create a Secret Question and Answer', once you create a secret question and answer - save it by selecting SUBMIT.
